Orientation Packet:    The packet consists of the following 8 documents in Part A & Part B. Print out all pages, fill them out and bring them to the Orientation Meeting on March 18th, 2015.  Packet Part A must be completed and returned in order to go participate at practice.

GSHH Program Registration.  Turn this in at the Orientation mtg.

We will be registering as a troop, so this information will be transferred to another form by the coaches.    

Permission-Practice -

Permission slip for practice at CAB April & May (2 pages)

Health Form - GSHH -

Girl Scouts Heart of the Hudson standard Health Form (2 pages)

PART B:

  

Permission-Training trip -

Permission for the weekend of river training and camping on a river. (2 pages)

Permission-Regatta -

Permission for racing & canoeing at the Race Canoe Regatta & Permission for camping at Camp Amahami in     Sanford, NY. (2 pages)

Race Registration 2015

Print out the form (2 pages). Parent must read the waiver (pg.2) and fill in only, the canoer’s name, birthdate, & parent signature.  Team info will be filled in later.

Photo / Transportation release -

Photo release for any photos taken during Kamikaze events and Parent agreement for transporting any Girl Scout  

Trip Agreement Form  -

Code of Conduct for both Participants and Adult volunteers on a Girl Scout Trip

Girl Scout Registration -

If registering for the first time, you can pick up a 2014 GS Registration form at the meeting bring $15.

Re-Registering? You can do it yourself online at GSHH 

Adult  GS Registration -

Parents are encouraged to be registered to be covered by the Girl Scout insurance, especially if they are driving girls at any time.
